Free Advance Health Care Directives and Guardian Nominations for Minors

With the COVID-19 pandemic sending us all into uncertain terrain, I wondered how I could make a positive difference in the lives of the people in my community. I know that fear can be a powerful motivator, but I also know that having peace of mind is invaluable. So that's what I hope to offer to you: peace of mind in knowing that your health care choices for yourself are honored no matter what; and, if you have minor children, peace of mind knowing that your children will be with the people you choose.

I am providing California Advance Health Care Directives and Permanent Guardian Nominations for Minor Children to you and your loved ones. Just choose which you need, follow the instructions, and I will draft these documents for you absolutely FREE of charge. And while they are only part of a comprehensive estate plan, these days, they may feel like the most important parts to have.

California Advance Health Care Directive

The California Advance Health Care Directive (AHD) enables you to make decisions about what you would want to happen if you became seriously ill and it also allows you to name who you would want to make health care decisions for you if you could not make your own decisions. This is sometimes called a "Living Will" and there also used to be a "Power of Attorney for Health Care." California has combined this two documents into the Advance Health Care Directive.

You will also want a HIPAA waiver so that your loved ones (those you name) can have access to your medical records and get information from your health care providers.

Here's how to get started:

  1. Read this Letter of Limited Engagement and accept the Terms of Engagement.
  2. Complete the AHD Questionnaire.
  3. I will reach out to you if additional information is needed.
  4. Receive and follow instructions to complete your AHD.

Nomination of Guardian for Minor Children

A Nomination of Permanent Guardian for Minor Children enables you to name who you would want to take care of your children if you were no longer able to do so. 

Here's how to get started:

  1. Read this Letter of Limited Engagement and accept the Terms of Engagement.
  2. Complete the Guardian Nomination Questionnaire.
  3. I will reach out to you if additional information is needed.
  4. Receive and follow instructions to complete your Guardian Nomination.

Please remember that these are only two parts of a comprehensive estate plan, but I'm hoping they will at least provide a little bit of peace of mind during these unusual times.
